REFERRAL.

WHO CAN BE REFERRED?

A professional referral may be suitable when the young adult:

• Is aged 18-26.

• Can access our Sutton, Epsom, nearby or online events.

• Would benefit from social connection, confidence, routine, participation or general wellbeing support.

• Is interested in voluntary, non-clinical community activities.

• Knows about the referral and has agreed to THE PERCENTERS. contacting them.

• Can participate safely with ordinary event facilitation or reasonable adjustments.

No diagnosis is required. Young adults can attend while receiving professional support, waiting for another service or after support has ended.

SPEAK TO US FIRST IF…

• You are unsure whether a community group is currently suitable.

• A support worker, carer or trusted person may need to attend.

• Significant communication, sensory, mobility or environmental adjustments may be required.

• There is relevant safeguarding or risk information that requires planning before attendance.

• The person is approaching 18, over 26 or lives outside our usual area.

DO NOT USE THIS FORM FOR:

• An emergency or situation requiring an immediate response.

• Therapy, counselling, diagnosis or clinical assessment.

• Crisis intervention or statutory case management.

• Someone who has not agreed to the referral.

• A request for guaranteed one-to-one supervision.

Continue to use your organisation’s usual urgent-care and safeguarding procedures. If there is an immediate danger to life, call 999.

WHAT HAPPENS NEXT.

Once the referral has been sent, the young adult and professional can expect the following:

1. WE RECEIVE AND REVIEW THE REFERRAL.

We will check that we have the correct contact details, consent and enough information to support a safe first connection.

2. WE CONTACT THE YOUNG ADULT.

Jake or another member of THE PERCENTERS. team will contact the young adult using their preferred contact method. We aim to make initial contact within five working days.

The message will explain who we are, share our website and Instagram, and show them how to explore the events.

3. THERE IS NO PRESSURE TO ATTEND.

Receiving a message does not commit the young adult to attending anything. They can view our website, follow our updates and take as much time as they need.

They can ask questions, say they are not ready or decide that THE PERCENTERS. is not right for them.

4. THEY CHOOSE WHAT FEELS RIGHT.

The young adult can look through the current events and choose something that speaks to them. We can help them understand the options and what to expect.

The referral itself does not automatically book an event place.

5. THEY BOOK THEIR EVENT.

When they feel ready, the young adult can book online. A professional, support worker or trusted person can help them book if they would like support.

Any arrangements involving a support person should be discussed with us before the event.

6. THEY ATTEND IN THEIR OWN WAY.

They can join in, observe, take a break or leave. There is no pressure to share personal information or participate in anything they do not feel comfortable with.

7. WE CAN PROVIDE A BASIC REFERRAL UPDATE.

With the young adult’s agreement, we can let the referring professional know whether contact was made and whether they chose to engage, declined or could not be reached.

We will not routinely share private conversations or personal disclosures.

IMPORTANT

THE PERCENTERS. is not an emergency, crisis or clinical service. Existing professional, safeguarding and statutory responsibilities continue while the referral is being reviewed and after the young adult begins attending.

Introduce THE PERCENTERS. as free events where young adults aged 18-26 can meet and connect with others in their age range. Avoid leading with our charity status, as this can make the support feel more formal.

2. SHOW THEM THE VIBE.

Show or send them our website and Instagram, @the.percenters. Our Instagram Highlights show the different events and what to expect. They can follow our updates until they feel ready to attend.

3. HELP THEM CHOOSE AND BOOK.

Visit www.thepercenters.co.uk and scroll through the current events together. Let them choose an event that speaks to them, then support them to book online or send them the link so they can book themselves.
